The forms are in 'PDF' format. (The file is 150KB).
Once downloaded, please print a copy of the application form, and the Gift Aid form and Direct Debit form if you so choose,
and post the completed forms to the AHS at the address at the bottom of the application form.
If not already installed on your computer, you will need Adobe Reader to view and print the forms,
and this is available from the Adobe website, free of charge, by clicking on the Adobe Reader logo below.
Alternatively, application forms are availabe by post on request from our Head Office.
